Master Advanced Hyaluronic Acid Filler Techniques: Structural Injections at CIME Academy

At CIME Academy, we train aesthetic doctors who want to go beyond basic techniques and become true experts in hyaluronic acid fillers. Advanced procedures—such as structural injections in areas like the temporal fossa or the zygomatic arch—require a deep understanding of anatomy and the properties of the materials used.
That’s why our courses are designed to help each student develop the confidence, safety, and precision necessary to apply these techniques with natural and harmonious results.

In this article, we share part of the approach we teach in our courses, based on advanced reference procedures that are setting trends in non-surgical facial beautification.

The Power of a Well-Executed Structural Filler

Hyaluronic acid fillers are not just a volumizing technique, but a powerful tool for facial sculpting.
A clear example is the use of products like Aliaxin Essential Volume, known for its high G-prime, meaning strong lifting capacity and firmness.
Due to their high molecular weight and cohesiveness, these products are designed to achieve an effective lifting effect when correctly applied over bone, especially using a needle.

In the Advanced Hyaluronic Acid Filler Course at CIME Academy, we explain how to choose the right product and how to work on different tissue planes—superficial, mid, or deep—to create a rejuvenating and beautifying effect that respects the natural anatomy and expression of the face.

Key Injection Areas: From the Zygomatic Arch to the Temporal Fossa

One of the most fascinating topics we cover is the treatment of the temporal and malar region. The procedure often starts with Aliaxin Essential Volume to treat areas such as:

  • Temporal fossa
  • Zygomatic arch
  • Zygomatic eminence
  • Beautification point
  • Medial cheek

Later, Aliaxin Superior Volume can be used to define the jawline, mandibular angle, and prejowl sulcus, achieving global facial harmonization.

The Importance of Anatomy and the “Safety Square”

One of the main concerns among students is how to inject safely, especially in highly vascularized areas like the temples.
At CIME Academy, we teach how to identify the “safety square” in the temporal fossa—an anatomically defined zone with minimal arterial presence when working at the periosteal level.

The protocol includes a perpendicular technique, where the needle touches bone, combined with a minimum 3-second aspiration to confirm vascular safety. This type of precision and safety is thoroughly practiced during our hands-on training sessions.

Technical Details That Make the Difference

The recommended injection volume in the temple area ranges between 0.5 cc and 0.7 cc per side, depending on patient anatomy and asymmetry.
The injection must be slow, allowing gentle hydro-dissection of tissues to lift the temporalis muscle and deep fascia.

We also teach how to withdraw the needle slowly to create a tamponade effect, minimizing the risk of bruising. Finally, manual molding of the product—with backward and upward movements—helps reposition the temporalis muscle, creating an immediate lifting effect.

Immediate and Natural Results

A precise application in the temporal region can significantly improve eye opening, correct temporal hollowing, lift the lateral brow, soften the upper eyelid, and even enhance the appearance of the tear trough—all with just 0.5 cc of hyaluronic acid on bone.

At CIME Academy, students practice these procedures under the supervision of experts, learning every technical detail to deliver natural and safe results for their patients.
